Tailoring Care for High-Need High-Cost Patient

Improving the care of high-need, high-cost (HNHC) patients is a compelling national challenge and should be a high priority for the next federal administration. A variety of promising policy options can promote improved care for HNHC patients. Some can be accomplished through executive action. Others will require legislative change.
